Follow these instructions to file an insurance claim for your wig / medical hair prosthesis.

Sorry we do not accept insurance, but we can help by providing you an estimate for your insurance provider. Download a sample estimate or contact us for a custom estimate. Make sure to ask your insurance provider how much they will reimburse you for your ‘medical hair prosthesis’, and how long it will take to get reimbursed.

Step 2: Purchase a Wig

We offer complimentary consultations. Due to limited availability, we require a $50 no show fee. To avoid this fee, please notify us within 24 hours to change/cancel your appointment. Then when you are ready, you can purchase a wig. Each wig is styled by our talented and compassionate wig stylists that will help you look and feel like yourself again.

Download and send Claim Form #1500 to your insurance company (contact your provider if the form is not listed below). Other items you will need include:
- Federal Tax ID: #13-3925124
- National Provider ID: 1992002042 (NPI Registry)
- License #: 045279
- Common procedure codes: Synthetic (A9282, 99199), Human Wig (L8499)
- Receipt: Paid in full
- Rx: Diagnosis code from doctor for a medical or cranial hair prosthesis
